Anyone can appeal his or her excommunication to the First Presidency (through his or her bishop or stake president).
So, the very people who excommunicated you in the first place have to approve your appeal? Yep, there's no way that could go wrong.

No. If you believe you were treated unfairly by your bishop you can appeal to the stake president. If you feel your stake president has treated you unfairly you can appeal to the First Presidency. The First Presidency wants you to first go to your bishop or stake president. However, you don't need bishopric or stake presidency approval to go to the next higher level. No one has to approve your appeal.

You knew that didn't you? I thought everyone knew that. Just common sense.
